Make every moment matter with us

At Community Hospice, we believe in making every moment count for those in our care. Every day, our team brings compassion, expertise, and a deep commitment to supporting people with life-limiting illnesses. Our work is about creating precious moments, offering unwavering support, and helping people live fully, even in the most challenging of times. If you’re looking for a meaningful career, where your leadership will truly impact the lives of patients and their families, then we would love to welcome you to our team.

About the role

We’re looking for a passionate, experienced, and inspiring team leader to join our hospital palliative care team. Based at Queen Elizabeth Hospital, Woolwich, and working collaboratively with both Lewisham and Greenwich NHS Trust and colleagues at the Community Hospice, you’ll play a crucial role in delivering responsive, high-quality palliative care. Whilst our hospital team is well established, the role of the team leader will remain clinical but with a high level of leadership duties and responsibilities. The role will require collaboration with senior staff within the QEH and their peers at the Community Hospice to streamline services, providing expert clinical advice and supporting timely and safe discharges out of the hospital setting.

As team leader, you will:

  • Lead and model exceptional care: deliver personalised, patient-centred care and be a role model for best practices in palliative care.
  • Support and develop the team: supervise, mentor, and empower team members, fostering a culture of continuous learning and professional growth.
  • Ensure operational excellence: oversee the service’s budget, prioritise referrals, and manage a responsive, seven-day-a-week care provision.
  • Champion collaboration: work closely with multidisciplinary teams across the hospital, hospice, and community to provide seamless, compassionate care.
  • Drive quality and innovation: lead quality assurance, contribute to research, and continuously seek ways to improve palliative care practices.

About you

You’ll bring a blend of clinical expertise, leadership skills, and a genuine passion for palliative care. We’re looking for someone who:

  • Holds a professional healthcare qualification (e.g. nurse, allied health professional, social worker) and has significant experience in a senior role within palliative care.
  • Has proven leadership and team management skills, with a background in interdisciplinary collaboration and a commitment to patient-centred care.
  • Is adept at resource management and experienced with digital health systems (SystmOne, iCare) for streamlined, effective service coordination.
  • Brings compassion, resilience, and a commitment to upholding dignity, diversity, and respect in every interaction.

Why join us?

At Community Hospice, our people make all the difference. We’re committed to providing you with the support, resources, and flexibility you need to thrive. Our benefits include:

  • Family-friendly policies and flexible working opportunities
  • Access to the NHS Pension Scheme for eligible staff
  • Tailored induction programmes and ongoing professional development
  • Wellbeing initiatives designed by and for our staff
  • Blue Light Discount Card and discounts in our charity shops
