How to make a referral
To refer a patient to our services, please complete the Specialist Palliative Care Referral Form and email it to our Assessment and Coordination Team at: gbch.referrals@nhs.net
Sometimes, it’s helpful to talk it through to make sure you get it right the first time. Our dedicated team is on hand to guide you through the referral process and answer any questions. Please contact them on 020 8320 5837.
What happens next
We know how important it is to act quickly. Our multidisciplinary team reviews referrals within 24 hours, and we aim to get back to you with a decision right after that. For urgent referrals, we handle them with extra care and attention on an individual basis.
