Our outpatients clinics
We’re here to make sure your care is not just focused on your physical symptoms, but on your psychological, social and spiritual needs too. We’ll put you at the centre of your care, looking at what matters most to you as a person. We can help with:
- Advice on managing pain and other distressing symptoms, such as nausea, vomiting and breathlessness
- Complex emotional, psychological and social support
Advance care planning, which is an opportunity to discuss and consider your future care preferences - Opportunity to talk about the effect your illness is having on you and those close to you
- Information about other services and help that may be available to help you when making decisions
ACCESSING OUR CARE
Referrals are usually made by a health professional, such as your GP, hospital team or community nurse, or sometimes people refer themselves. Each referral is assessed by one of our senior specialist nurses, who will contact you or a nominated carer or family member (with your permission), to do an initial telephone assessment. This assessment is designed to look at and understand your individual needs, to make sure you receive the best possible care that’s right for you.
Depending on your individual needs, support will be provided through a mixture of
- Outpatient clinic appointments
- Telephone or video appointments
- Home visits for those not well enough to travel

Please note, parking is free but limited at the hospice and there is a paid car park at the hospital.
Our friendly, highly experienced and skilled team of nurses are looking forward to meeting you and are here to help with what matters most to you. They have extensive knowledge of other services in the local area and can help you navigate them based on your needs.
As an outpatient you will have access to our 24-hour, 365 days a year support and advice service.
The specialist palliative care team provide a telephone service for anyone requiring palliative care advice or support. This service can be used by patients as well as carers, friends or family members, though please note that support from 5pm to 9am is for urgent concerns.
Any information you share with us is stored on your electronic patient records here at the hospice, and we’ll update your GP who remains responsible for your care. We can also liaise with your hospital teams/specialists if required.
